New Student 

Step 1: Visit www.chpstudenthealth.com and
search for your school's page.

Step 2: Select 'ID Cards' from the top navigation
menu.

Step 3: Select 'Create New Account' in the
Student Sign On area.

Step 4: Complete the Create New Account.
Information needed to create account:

• School ID
• Last Name
• Date of Birth
• E-mail Address
• Password

Step 5: Download or print ID card.

Returning Student

Step 1: Visit www.chpstudenthealth.com and
search for your school's page.

Step 2: Select 'ID Cards' from the top navigation
menu.

Step 3: In your account, visit “Student Options” at
the top.

Step 4: Click on “Print ID card.”
