Please submit all work order requests for Physical Plant through our online request system. 

Submit Work Order

Instructions on How to Fill Out the Work Order Form:

1. Select Guest Log-In option
2. Select Service from drop-down arrow. Enter the most logical choice from the list of shops. If there is a question as to which shop is required, select Misc. and we will edit the service at Physical Plant. NOTE: All request for residence halls should be listed as Res-Life for service.
3. Select building from drop-down arrow.
4. Select asset from drop-down arrow (if asset cannot be found, select the building name again, which is included in the list of building assets).
5. In description section, please provide as much information as possible (including the time the room is available and phone ext. and/or cell phone).
